What medical documents do you need to have for registration at a school or preschool educational institution (daycare, preschool)?

During the registration process, it is necessary to provide documentation on immunization - confirmation of received vaccinations. The rules of the Ministry of Health of the country from which the child came and the local legislation of the state where the student will be enrolled in the school are taken into account. The main document is the Certificate of Immunization Status (CIS).

How to get and where to submit immunization documents?

The CIS is handed over to the employee, medical worker of the educational institution responsible for the registration process.

If the child has health insurance, that is. visits a doctor and has all the necessary vaccinations, the employee of the educational institution independently prints the CIS from the Immunization Information System (IIS) database. In the future, the educational institution will monitor the vaccination schedule.

Is immunization a mandatory condition for entering school or kindergarten?

Immunization is not a mandatory condition for admission to school or kindergarten. If the child does not have documentation of vaccinations, a completed certificate of exemption from vaccinations for religious or other reasons must be provided.

What to do if there are no vaccination documents?

If the child does not have vaccination records, then at least the first dose of the catch-up series will be required and subsequently a completed Certificate of Immunization Status (CIS) or a completed Immunization Exemption Certificate must be provided. Pupils who need to make up for missed vaccinations will be given a vaccination schedule and will need to adhere to the deadlines specified in this schedule. At the same time, the student has a conditional status of admission to school.

The schedule "converts" vaccinations for conditional admission to classes at school or preschool, the child must receive all doses of the vaccine before the beginning of the educational process. Conditional admission lasts 30 days, and during this period vaccination must be carried out, otherwise the child will not be admitted to classes.

How to properly translate documents according to the requirements of the state of Washington?

How can you confirm the presence of the PDA vaccine if it is not included in the vaccination card?

Is confirmation of the tuberculin test required for adults and children for admission to college/work?

Yes, some educational institutions and employers may ask about the availability of a person's TB test. The check will be carried out through a general blood test. However, people who have had a vaccine such as BCG may experience a false positive. This is a normal phenomenon and should not be worried. In this case, you need to take a blood test called the Quantiferon Gold Test (QFT), which will help detect the bacteria that causes tuberculosis.

What vaccinations are required to apply for a green card?
