This website Vax4school contains basic information about vaccines for children for applications to childcare or schools.
The website provides translation service for vaccine records, free of charge. A parent (legal guardian) can upload scans of national vaccination records (Ukrainian, Russian languages accepted). The translation version will be emailed in about a week..
Due to privacy regulations, we suggest that school or daycare personnel do not use the website directly, but rather refer parents to the website Vax4school.
We encourage legal guardians and nurses to work together to enter accurate information into the state immunization information system (IIS).
